In order to obtain a Polish foreigner's travel document (PDP) or a provisional Polish foreigner's travel document (TPDP) or a Polish foreigner's identity document (PDTC), you should submit:

• 1 copy of a completed form (download the form PDP , TPDP , PDTC )
• a 35 x 45 mm up-to-date photograph of the person applying for the issue of the document and persons covered by the application (the photograph should show the person looking straight ahead, without a headdress or glasses with dark lenses, with eyes open and not covered with hair, with a natural face expression and closed mouth and face being evenly illuminated; a person with a sight defect, wearing glasses with dark lenses may attach photographs of her/him wearing glasses with dark lenses; a person wearing a headdress due to the requirements of her/his religion may attach photographs of her/him wearing a headdress, but the headdress may not prevent the identification of the person)
• a document confirming your identity (You are exempt from the obligation of having a valid travel document or a document confirming your identity if you have a certificate that you are a victim of human trafficking and you do not have such a document and cannot obtain as a result of special circumstances)
• documents that are necessary to confirm the data and circumstances provided in the application (e.g. documents confirming the inability to obtain a travel document of the country of origin, documents confirming the status of a stateless person, a decision on covering with subsidiary protection on the territory of Poland or a residence permit for humanitarian reasons)
• the confirmation of permanent or temporary registered residence
• the confirmation of payment of the stamp duty

If you are applying for the issue of a Polish travel document for a foreigner or a Polish temporary travel document for a foreigner with an entry on minor children or persons subject to such entry, you must additionally submit:

• an application (download the form PDP , TPDP ) completed only in parts A and C (for each person that is being entered)
• an up-to-date photograph of the person whose data is to be entered in the document (the requirements concerning the photograph are the same as above)
• a document confirming the child's identity
• a document confirming the degree of kinship with the applicant

When submitting the photocopies of the required documents, you should also present the originals.
