Right of permanent residence of an EU citizen
When should I replace the document confirming the right of permanent residence of a European Union citizen?
A European Union citizen should report to the voivode having jurisdiction over her/his place of residence in the event of:
- the change of data provided in it,
- damage or occurrence of other circumstances making it impossible to establish the card holder's identity.
In the event of loss of the document confirming the right of permanent residence, a new document is issued.
REQUIRED DOCUMENTS:
- an application for the issue or replacement of a new document confirming the right of permanent residence;
- a valid travel document or another document confirming one's identity and citizenship;
- 2 non-damaged, colour 4.50 x 3.50 cm photographs showing the person without a headdress or glasses with dark lenses, with her/his head posed in left half-profile showing her/his left ear and with her/his face being evenly illuminated;
- a confirmation of residence registration, only if it is a permanent or temporary registration for more than 3 months;
- a document confirming the right of permanent residence of an EU citizen - in the case of its replacement.
