When should the residence card of an EU citizen's family member be replaced?
In the event of:
- the change of data provided in it,
- damage,
- the change of its holder's appearance making it impossible to establish her/his identity,
- expiry (only if the residence card of an EU citizen's family member was issued for the period of 5 years. For residence cards issued for a period shorter than 5 years, one should submit an application for the issue of a residence card of an EU citizen's family member).
In the event of loss of a residence card of an EU citizen's family member, a new card is issued.
Documents required for the replacement/issue of a residence card of an EU citizen's family member:
- an application for the replacement or issue of a new residence card of an EU citizen's family member,
- a valid travel document,
- 2 not damaged, colour 4.50 x 3.50 cm photographs showing the person without a headdress or glasses with dark lenses, with her/his head posed in left half-profile showing her/his left ear and with her/his face being evenly illuminated,
- a confirmation of residence registration, only if it is a permanent or temporary registration for more than 3 months,
- a residence card of the EU citizen's family member;
