A family member of a European Union citizen should report to the voivode having jurisdiction over your place of residence in the event of:

• the change of data provided in the card,
• damage or occurrence of other circumstances making it impossible to establish the card holder's identity,
• expiry.

In the event of loss of a permanent residence card of a European Union citizen's family member, a new card is issued.

REQUIRED DOCUMENTS:

• an application for the replacement or issue of a new permanent residence card of a European Union citizen's family member,
• a valid travel document,
• 2 non-damaged, colour 4.50 x 3.50 cm photographs showing the person without a headdress or glasses with dark lenses, with her/his head posed in left half-profile showing her/his left ear and with her/his face being evenly illuminated,
• a confirmation of residence registration, only if it is a permanent or temporary registration for more than 3 months;
• a document confirming the right of permanent residence or a permanent residence card of a European Union citizen's family member - in the event of a replacement.
