the application may only be submitted in person, and not through a representative. The requirement of personal submission of an application does not apply no a minor;
The permanent residence card of an EU citizen's family member may be collected by a representative;
the application should be completed in Polish, legibly, with capital letters;
the original documents should be presented for inspection at the office;
if you attach to the application documents in a language other than Polish, you should also attach their translations into Polish made by a Polish sworn translator or consul.
