Permanent residence card of an EU citizen's family member

What documents do I have to submit?

  • an application for the issue of a permanent residence card of an EU citizen's family member,
  • a travel document,
  • 5 recent colour photographs, undamaged, size 45x35 mm, made in the last six months on a plain white background, with good focus, clearly showing the eyes and face from the top of the head to the top of the shoulders; the face should occupy 70-80% of the photograph; the photograph should show the person looking straight ahead with their eyes open, not covered by hair, with a natural facial expression and mouth closed

    Aliens with congenital or acquired vision defects may attach a photograph showing them wearing glasses with dark lenses.
    Aliens wearing a headdress in accordance with the principles of their religion may attach a photograph in the headdress, as long as the face is fully visible.

  • documents confirming a 5-year continuous residence on the territory of the Republic of Poland together with an EU citizen,
  • a document confirming the right of residence on the territory of the Republic of Poland of an EU citizen, e.g. a certificate of an EU citizen's residence registration or a document confirming the right of permanent residence of a European Union citizen, 
  • a confirmation of residence registration, only if it is a permanent or temporary registration for more than 3 months.

 
One should additionally submit:

  • a spouse – a document confirming conclusion of the marriage (e.g. marriage certificate),
  • a descendant – a document confirming the existence of family bonds and a document confirming the age or certifying that the person is the European Union citizen's or its spouse's dependent (e.g. birth certificate),
  • an ancestor – a document confirming the existence of family bonds and a document certifying that the person is the European Union citizen's or its spouse's dependent.
