Recognition of the repatriate

Which documents do I have to submit?

  • The foreigner applying for recognition as repatriate, submits the following documents:
  1. application for recognition as repatriate, with up-to-date, high-definition photograph, size 5 cm x 4 cm
    application
  2. decision of the consul to be recognized as a person of Polish ancestry. Person, who does not have such a decision, encloses documents confirming Polish ancestry with the application, in order for the consul to render the decision to be recognized as a person of Polish ancestry,
  3. certified photocopy of valid document confirming identity and citizenship,
  4. original version of the extract from the Birth Certificate, issued by Polish Registry Office,
  5. original version of the extract from the Marriage Certificate or other document, specifying the marital status,
  6. CV,
  7. certified photocopy of the Temporary Residence Card,
  8. confirmation, that the person has studied at a university pursuant to regulations on beginning and continuing studying by persons not being Polish citizens, as well as the copy of the University Certificate,
  9. confirmation that before the date of coming the Act into force, the applicant had resided permanently on the territory of present Republic of Armenia, Republic of Azerbaijan, Republic of Georgia, Republic of Kazakhstan, Republic of Kirgizstan, Republic of Tajikistan, Republic of Turkmenistan, Republic of Uzbekistan or the Asian part of the Russian Federation,
  10. statement on whether the applicant acted to the detriment of the vital interests of the Republic of Poland or participated or is participating in the violation of human rights, during stay outside the borders of the Republic of Poland.
