Confirmation of possessing or loss of Polish citizenship
Conditions I have to meet
- Documents enclosed with the application have to be submitted either in the form of original version or certified copy.
Who can certify that the document is a true copy?
1. Polish notary, Polish consul or the applicant’s proxy, being an attorney, legal advisor, patent lawyer, or tax advisor,
2. the documents can be certified at the Mazowieckie Voivodship Office in Warsaw upon submitting the application. To this end the original version of the document and photocopies made on one’s own have to be prepared.
- All the documents and official letters in foreign language have to be translated into Polish by Polish consul or Polish sworn translator.
- You can appoint a proxy to run your case. Proxy has to be enclosed with the application together with the proof of administrative fee payment (application 3).
- If you live abroad and you have not appointed a proxy to run the case, you should appoint a process agent in Poland, i.e. a person, at whose address all the correspondence will be posted (application 4). Being a process agent shall not be subject to payment of a fee. If there is no process agent, all the official letters and decisions are left in the case files and are deemed to be delivered.
