The applicant, who applies for the confirmation of possession or loss of Polish citizenship, submits the following documents:

• application for the confirmation of possession or loss of Polish citizenship,  (application 1 and application 2 )
• proof of the payment of administrative fee,
• certified photocopy of current ID document (e.g. valid ID card, valid passport),
• certified copies of vital records, i.e. Birth Certificate, Marriage Certificate, other documents confirming data and information included in the application (e.g. invalid Polish passport, Military Service Book issued by the Polish authorities, invalid Polish ID card, certificate confirming that one of parents has Polish citizenship, change of name or surname deed, document confirming naturalization as a foreign citizen, proof of the last place of permanent residence in Poland with a note on issued ID card).
